Student Declaration

Letter to Placement Employer

Bring a print of this form to the school principal on your first orientation day and upload the signed form along with the Student Declaration to Pepper before practicum begins.

  • You'll upload it to the folder entitled "WSIB Forms", which can be found inside the "special forms" folder on your pepper community page for CTEP. 
  • When uploading the document please name it as such: LAST NAME,FIRSTNAME-WSIBCov-201718 or  LAST NAME,FIRSTNAME-Declaration-201718, respectively.

Worker Health & Safety in 4 Steps [All CTEP teacher candidates are required to complete this online learning module and retain a digital copy of the certificate of completion for their records.]

          Criminal Record Check/Vulnerable Sector Search

Criminal record checks are conducted by the police service where the teacher candidate lives. Information on obtaining a CRC/VSS can be accessed on the OISE Registrar’s Office website.

Teacher candidates find out their practicum placement and later access their official summative evaluations by logging into the Pepper database. Please download and save back-up files of your summatives as you only have access for a limited time after you graduate.
