Graduate Assistantship Application Form
OISE offers financial assistance to students in the form of graduate assistantships, which have a work requirement involving educational research to support programs of full-time graduate study. Please see student services for more details.

Registrar's Office Request Forms
Please see the registrar's office request forms site in the graduate studies section for forms such as Degree Extension, Request for Leave of Absence,
Course Add/Drop, Individual Reading and Research Course, and many more.

Muriel Fung Nomination Form
The Muriel Fung Student Appreciation Award recognizes graduate students who have made outstanding extra-curricular contributions to OISE.  The nomination form must be submitted to the GSA office, 8-105, or electronic copies to vpgsa.oise@utoronto.ca. 

GSA Executive Nomination Form
A new GSA executive is elected each year by OISE graduate student nominees.
