APPLICATION HANDBOOKS & FORMS

During the application period, the OISE Application Handbooks, Profiles and other important documents required to complete your application are available from this page.

Our forms are only available during the application period; however, the Application Handbook can be accessed year-round. Note that after the application deadline and prior to the next September's release of application materials, the Application Handbooks available from this site pertain to the previous application cycle. New copies of our Handbooks and supplementary forms are posted to this website each September on the application release date.

► OUAC/TEAS APPLICATION (Basic application form for the CONSECUTIVE BACHELOR OF EDUCATION and TECHNOLOGICAL EDUCATION program)
OUAC/TEAS	The basic application form -- TEAS Online Application -- is submitted via the Ontario Universities Application Centre (OUAC) Teacher Education Application Service (TEAS) . It is available during the application period from the OUAC/TEAS website. It is a common application form used by all one-year initial teacher education programs in Ontario (i.e., this is the basic starting point to apply to any such program in Ontario).
U of T TEAS Information	The University of Toronto section of the TEAS application booklet that contains instructions and information necessary to complete your application. All applicants (Primary/Junior, Junior/Intermediate, Intermediate/Senior and Technological Education) are advised to download this file.

► CONSECUTIVE BACHELOR OF EDUCATION SEE ALSO BASIC OUAC/TEAS APPLICATION ABOVE
Online Applicant Profile	The OISE Online Applicant Profile and also the Demographic Form and the English Proficiency Form. Applicants to the Primary/Junior, Junior/Intermediate and Intermediate/Senior divisions must complete this online form (see also the Application Handbook below) A PDF sample of the Online Profile is available for viewing on the entry page for the Online Profile. In order to actually enter the Online Profile site you will need to have the User ID that you are provided with when you create a TEAS online application account and the OUAC Reference number that you receive after you have submitted the OUAC/TEAS application.
	The OISE Bachelor of Education Application Handbook that contains instructions and information necessary to complete your application. Applicants to the Primary/Junior, Junior/Intermediate and Intermediate/Senior divisions are strongly advised to download this file (and read it carefully).

►TECHNOLOGICAL EDUCATION SEE ALSO BASIC OUAC/TEAS APPLICATION ABOVE
Technological Education Profile No longer available.	The Technological Education Profile and Verification Form and also the English Proficiency Form. Applicants to the Technological Education program must download this file. (See also the Technological Education handbook file below) After you submit your Technological Education Profile, you will receive an email to confirm that we have received your Profile.
	The OISE Technological Education Application Handbook contains instructions and information necessary to complete your application. Applicants to the Technological Education program are strongly advised to download this file (and read it carefully).

Vari Scholarship Application	The application for the University of Toronto Vari Scholarship that is open to applicants to the Intermediate/Senior Division. Visit the Vari Scholarship page for detailed information about this $10,000 award.
