Please send to the OISE Registrar's Office by June 30th (May 15th for those in the Technological Apprenticeship Program).
 

Proof of Name and Date and Place of Birth

We no longer collect these documents, as you have already submitted the information on your application, however, if required for verification purposes we may ask for documentation, particularly if there is a name/legal status discrepancy.

You will be required to submit this documentation to the Ontario College of Teachers when you apply for your Teacher Certification.  More information can be found at www.oct.ca.

Legal Status in Canada

 
International students will be required to submit a valid student visa or other legal document.
 
NOTE: Any document in a language other than English or French must be accompanied by an official translation.  Translations will only be accepted from: the consulate, high commission or embassy to Canada of the country which issued the documents; a Canadian embassy, consulate or high commission in the country from which you emigrated; a translator accredited by a professional association of translators in Canada.  To find an accredited translator, consult the Association of Translators and Interpreters of Ontario.
 


Official Final Transcript Stating the Degree Has Been Conferred/Granted

(Technological Studies students who do not have a university degree are exempt from the following transcript requirement.)

Undergraduate degree and any pre-requisite course requirements must be completed by end of August 2013 and your degree must be conferred by the 2013 Fall Convocation in order for you to remain eligible to take our program. Students who do not fulfill this requirement may be withdrawn from the program. If the official transcript, which you submitted in support of your application for admission, shows that your degree has already been conferred or granted, then you do not need to submit another copy.  If the official transcript submitted in support of your application does not show that your degree has been conferred, then you must have official final transcripts submitted showing that your degree has been conferred.  (University of Toronto graduates are not required to submit transcripts.)
  
If your degree will not be conferred until the 2013 Fall Convocation, advise the OISE Registrar's Office, Initial Teacher Education - Admitted Students Unit, Room 4-455, 252 Bloor St. W., Toronto, ON, M5S 1V6, in writing by June 30, 2013.
 
Only official transcripts, bearing the institution's seal and received directly from the institution concerned will be accepted.  Transcripts stamped ‘Issued to Student’ are not acceptable.


Substantiation of A Change of Name


Please ensure that the name on your Offer of Admission is the complete name by which you are legally and correctly known, and is the same as on the documentation you provide for proof of Canadian Citizenship or other status in Canada.  Any changes to this name may require supporting documentation as outlined in the Statement on Personal Information on Academic Records and Corroborative Documents.  
 
As our program results in a recommendation to an external licensing agency, any discrepancy or variation in name on documents submitted, must be substantiated by a legal document such as a Marriage certificate (only for surname change due to marriage), or a Name-Change certificate issued by a Canadian province or territory or similar judicial authority.  Name discrepancies between transcripts or other documents and your legal name may require that you submit a sworn statement explaining the discrepancy.  You will be advised if any additional documentation is required to resolve a name discrepancy. Your name as it appears in the University’s official student records is the name that will appear on your diploma. You can view your name on www.rosi.utoronto.ca. The name that appears on the LOGIN page is how your name appears, not only in the official student record, but also on your diploma.

Note: the deadline to make changes/resolve discrepancies for diploma puposes is usually in early March.
