Transcripts and Records

AQ Transcripts

An Official Transcript of courses taken through the Additional Qualification Program for Teachers can be ordered through the following methods:

• print and complete the AQ Transcript request form and return it to us via mail or fax (be sure to include payment)
• a letter (including payment) can be mailed to:The Records Officer, OISE Registrar's Office, 252 Bloor Street West, Room 4-455, Toronto, Ontario M5S 1V6
• a fax sent to: (416) 323-9964
• email sent to: transcripts@oise.utoronto.ca

FAX and Email requests must have credit card (VISA or MasterCard only)information, including expiry date, otherwise your request will not be processed.

The fee for an A.Q. transcript, including applicable taxes, is $10.00 each. Cheques should be made payable to the University of Toronto.

The following information is required:

• Your name (previous if applicable)
• student number or birthdate
• the course(s) taken and the year(s)
• the address where the transcript should be mailed as well as your address for mailing of receipt.

Degree Transcripts

All requests for transcripts of degree credit courses, taken at the Faculty of Education, OISE or other Faculties must be made via the student web service: www.rosi.utoronto.ca or by contacting the:

Course Descriptions

Occasionally certification bodies require copies of pages from previous years B.Ed. calendars.  If required, these are available for a charge of $15.00, contact us for details  transcripts.oise@utoronto.ca
