The University of Toronto "Verification of Student Illness or Injury Form" must be completed by all students requesting special consideration based on illness or injury.  Guidance for completion is included on the form itself.  Students can download a copy of the form from the Student Services website or they can pick one up from Student Services.

OISE students requesting consideration are asked to arrange an appointment with Student Services (8th floor, 416-978-2277) to submit the form and to discuss their individual needs with an advisor.
