Verification of Completion

The Verification of Completion form must be submitted to OISE on the final day of the Internship. This form is required for each TC to complete the Internship, and in turn, complete the Initial Teacher Education Program at OISE. The Verification of Completion form is made electronically available to all Site Supervisors during the last week of the Internship. An email is sent to each Site Supervisor providing the instructions and timing for this electronic submission. The Verification of Completion form is electronically accessible to TCs after it has been submitted by the Site Supervisor.

Concern About a Teacher Candidate’s Performance

Site Supervisors who are concerned about a Teacher Candidate’s performance during the Internship must address these with the Teacher Candidate upon observation. The OISE Advisor should also be notified of the concerns. Notification of concerns must be documented by the OISE Advisor and communicated with the Academic Internship Coordinator. A plan of action must be developed with the Teacher Candidate, outlining areas of concerns, as well as expectations in order for the Teacher Candidate to have a successful Internship. Teacher Candidates and/or Site Supervisors should contact the OISE Advisor, or the Academic Internship Coordinator with further questions.

Unsuccessful Completion & the Appeal Process

Successful completion of the Internship is a pre-requisite for graduation of the Consecutive B.Ed./Dip. Tech Ed. Programs. Teacher Candidates who receive a "Not successful" rating on the Verification of Completion may appeal the result using the same appeal process applied to the practicum. Questions regarding the appeal process should be directed to the Academic Internship Coordinator in SUPO. Subject to review and approval by the Dean’s Review Committee, unsuccessful candidates will be provided with one opportunity to successfully complete a make-up Internship. Two unsuccessful Internships will result in the Teacher Candidate failing the B.Ed./Dip. Tech Ed. Program, and not being recommended to the Ontario College of Teachers for the Certificate of Qualification.
