Student forms

We are transitioning our forms to an online format. The e-signature capability is powered by DocuSign. This will require you to provide an email address for each signer. There is a version of each form for master's and doctoral students; please use the appropriate version.

Some of our forms must be approved by an instructor. Please contact the instructor before starting the form. You can find their email address on our course schedule (if it is not available there, please contact the department offering the course).

Registration

 

 

 

 

 

  • Request to Re-register  For full-time (not flexible-time) PhD students who started their program 1 September 2010 or later, and students in all other master's and doctoral programs who started their program 1 September 2011 or later.

 

  • Reinstatement after expiration of time limit  Only for students who have not opted-in to the current degree regulations, and who are full-time (not flexible-time) PhD students who started their program before September 1, 2010, or are students in any other master's and doctoral programs who started their program prior to September 1, 2011.

 

  • Program extension request (current regulations) form  For full-time (not flexible-time) PhD students who started their program 1 September 2010 or later, students in all other master's and doctoral programs who started their program 1 September 2011 or later, and students who started their program before this and wish to opt-in to the current degree regulations.

 

  • Program extension request (prior regulations) form  Only for students who have not opted-in to the current degree regulations, and who are full-time (not flexible-time) PhD students who started their program before September 1, 2010, or are students in any other master's and doctoral program who started their program prior to September 1, 2011.

Letter request

If you are requesting a letter for: proof of completion of your degree, registration status, or for legal status in canada purposes, please complete and return the Ancillary Fees Charge Form.

Certification of degree

A certification of degree is available through the Office of Convocation

Course descriptions
Occasionally, teacher certification bodies require copies of pages from previous calendars. These are available for $17.00. Please contact transcripts.oise@utoronto.ca.

 

Documentation in support of legal proceedings *  
A copy of the student file can be provided to Lawyers upon receipt of a written request, which must include duly executed authorization signed by the student/former student. Our file retention schedule depends on the program of study.  The fee is $50, payable by Cheque to the University of Toronto.

*PLEASE NOTE: student files are paper and we cannot fulfill requests until such time as we are advised to return to the office after the COVID-19 outbreak.


Additional qualification (AQ) transcripts
An official record of additional qualification courses is available for $12.00. 
 

  • For courses completed in Winter 2017 or later:

  • For courses completed in Fall 2016 or earlier:
    Complete and the return the AQ transcript request form,* along with payment, to
